Key Responsibilities\n

As an LCSW at Great Lakes Consultants Services, your responsibilities will include:

• Engaging with community organizations to facilitate connections for patients post-discharge, thereby promoting continuity of care.

• Documenting all interactions and interventions accurately and timely in accordance with organizational policies and legal standards.

• Participating actively in case consultations, hospital committees, and program development initiatives focused on enhancing social work practices.

• Providing crisis intervention services to patients and families experiencing acute distress or suicidal ideation.
