P alliative (Hospice) Medicine NP / PA

Join a integrative non-profit integrated regional health system in NW Ohio (outside of Toledo). Suburban community offering a quality of life with affordable housing and lower cost of living.

Expanding inpatient Palliative Care inpatient service. Hospital-employed position. Currently 2 NPs.  Support 6 patients/day. Monday - Friday (8am-4:30pm/5pm). Focus on symptom management, care coordination, transitions of care, and ensuring treatment decisions align with each patient's goals and values. Assist with discharge planning, including transitions to home-based palliative care or hospice.  Call shared 1:3 (with 2 other NPs), evening and weekend rotation (only phone consults). Cerner EMR.

Requirements:

• Board Certified: Family Medicine or Internal Medicine
• Prefer Acute Care NP certified, or willing to obtain
• Palliative Medicine/Hospice experience
• OH license, or eligible

Compensation:

• Guaranteed base salary to $135K
• Productivity Incentives
• Signing bonus (up to $10K)
