Pay & Benefits

• Pay Rate: $60.30/hour

What You'll Do

• Evaluate and develop treatment plans for patients in home health settings
• Implement therapeutic interventions to improve patient mobility and function
• Educate patients and families on therapeutic activities and home exercises
• Document patient progress using Meditech Home Care EMR system
• Coordinate care with physicians and other healthcare professionals
• Support department goals as an educator and team collaborator
• Travel within the 50-mile radius of Shreveport to patient locations as needed

What You Need

• Required: Valid Louisiana Physical Therapy license, BLS certification
• Preferred: Outpatient or home health experience, familiarity with Meditech EMR
• Key Skills: Strong interpersonal skills, ability to work independently, adaptable to travel
• Other: Ability to manage a diverse caseload and perform comprehensive assessments
