We are seeking a compassionate, experienced, and clinically strong Family Nurse Practitioner (FNP) to join our growing clinical team. This is an opportunity to provide comprehensive primary care while developing meaningful long-term relationships with patients and providing clinical oversight within our virtual RPM and CCM programs.

Important Location Requirement

THIS IS A REMOTE/VIRTUAL POSITION; HOWEVER, THE PROVIDER MUST RESIDE WITHIN THE CITY OF SEATTLE. PROOF OF SEATTLE RESIDENCY IS REQUIRED, NO EXCEPTIONS.

The position may be performed remotely from the provider's residence, with flexible scheduling based on patient and organizational needs.

Position Overview

This is a 1099 independent contractor position in which the FNP will serve as a primary clinical point of contact for new and established patients.

The FNP will conduct initial patient visits, perform comprehensive assessments, establish patient-provider relationships, and enroll appropriate patients into RPM and CCM programs. The provider will then maintain clinical oversight of patients participating in these programs while collaborating with care coordinators, clinical staff, physicians, specialists, and other members of the healthcare team.

Clinical staff will manage routine patient monitoring and monthly CCM follow-up activities under the FNP's general clinical supervision. The FNP will be responsible for reviewing escalated patient-generated health data, evaluating clinical concerns, intervening when necessary, and modifying treatment or care plans as clinically appropriate.

In addition to chronic care responsibilities, the FNP will provide comprehensive primary care services, including evaluation and management of acute and chronic conditions, medication management, preventive care, patient education, and coordination of specialty services.

Key Responsibilities
Provide comprehensive primary care services to patients across the lifespan, from pediatrics through geriatrics.
Conduct new patient and follow-up visits, comprehensive health assessments, and physical examinations.
Establish strong, ongoing patient-provider relationships through patient-centered virtual care.
Develop individualized treatment plans based on patient history, examination findings, diagnostic results, and clinical needs.
Diagnose and manage acute and chronic medical conditions, including hypertension, diabetes, cardiovascular disease, respiratory illness, and other common primary care conditions.
Enroll appropriate patients into Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.
Provide clinical oversight for patients enrolled in RPM and CCM programs.
Provide general clinical supervision and guidance to care coordinators and clinical staff supporting patient care.
Review escalated patient-generated health data, including blood pressure, blood glucose, weight, pulse oximetry, and other relevant measurements.
Assess patient escalations and intervene clinically when indicated.
Adjust treatment plans and make appropriate clinical recommendations based on patient status and monitoring data.
Order and interpret laboratory tests, diagnostic studies, and imaging.
Prescribe medications and manage pharmacological therapies in accordance with Washington State laws and regulations.
Monitor medication effectiveness, adherence, and potential adverse effects.
Collaborate with physicians, specialists, care coordinators, and other healthcare professionals regarding patient care.
Coordinate referrals, transitions of care, and follow-up with specialists and other healthcare providers.
Educate patients and families regarding preventive care, wellness, chronic disease management, medications, lifestyle modifications, and treatment adherence.
Educate patients regarding proper use and adherence to RPM devices and technology.
Promote patient engagement and adherence to individualized care plans.
Review and respond to clinical escalations in a timely manner.
Maintain accurate, complete, and timely documentation in the electronic health record (EHR).
Ensure clinical documentation supports applicable CMS requirements for RPM and CCM services.
Support accurate and compliant billing practices related to RPM and CCM.
Maintain patient confidentiality and comply with HIPAA and applicable healthcare regulations.
Participate in a collaborative virtual clinical environment while maintaining appropriate independent clinical judgment.

Required Qualifications
Active, unrestricted Family Nurse Practitioner (FNP) certification through AANP or ANCC.
Current, unrestricted Washington State ARNP/NP license, in good standing.
Current underlying RN license in good standing, including Washington Compact eligibility as applicable.
Master's or Doctoral degree in Nursing (MSN or DNP) from an accredited nursing program.
Current national board certification through AANP or ANCC.
Active DEA registration and prescriptive authority in Washington State.
At least 1 year of experience in primary care, family practice, internal medicine, outpatient care, or a closely related clinical setting.
3–5 years of NP experience preferred.
Demonstrated experience managing chronic conditions such as hypertension, diabetes, and cardiovascular disease.
Active Medicare enrollment or ability to obtain Medicare enrollment.
No OIG or GSA exclusions.
Must reside within the City of Seattle and be able to provide proof of residency.
Preferred Qualifications:
Previous experience with Remote Patient Monitoring (RPM).
Previous experience with Chronic Care Management (CCM).
Prior telehealth or virtual-care experience.
Experience working with Medicare patients and CMS requirements.
Experience supervising or providing clinical oversight to care coordinators or clinical staff.
Strong familiarity with chronic disease management.
Experience working with EHR systems and virtual healthcare platforms.

The exact starting hourly rate offered will be determined at the time of candidate selection. Compensation will be based on a variety of factors, including clinical experience, years of NP practice, education, certifications, credentials, licensure, skillset, and relevant experience with primary care, telehealth, RPM, and CCM.

As a 1099 independent contractor, the provider is responsible for their own applicable taxes, insurance, and other obligations associated with independent contractor status. This position does not include traditional employee benefits.
