Working part-time on weekends, the Maryland Licensed Case Manager RN will coordinate discharge planning and interventions in collaboration with the clinical team, focusing on patient wellness and efficient health service utilization. Key responsibilities Manage discharge planning and coordination for patients with complex health needs Collaborate with the clinical team and medical providers to promote improved care outcomes Strive to minimize payment denials through efficient utilization of health services Required qualifications Active Registered Nurse License in Maryland Bachelor's degree in nursing (BSN) from a CCNE accredited program 5-7 years of relevant nursing experience Prior experience in care management or related fields is preferred Knowledge of discharge planning processes and health service utilization Virtual Vocations Inc
