Job Description:

• Under supervision of an RN, performs a head-to-toe assessment on all patients and re-assesses as per policy.
• Provides direct care and indirect patient care services that meet psychosocial, physical and general aspects of care.
• Revise plan of care as indicated by the patient's response to treatment.
• Assists in formulation of a teaching plan based on identified patient learning needs and evaluates effectiveness of learning.
• Assists physicians with procedures and performs services requiring technical and manual skills under the direction of the RN.
• Meets current documentation standards and policies.
• Communicates with Social Services regarding patient needs.
• Knowledge of medications and their correct administration based on age of the patient and their clinical condition.
• Ability to perform treatments and provide services according to scope of practice.

Requirements/Qualifications:

• Current LPN licensure in New York State
• BCLS required
• Bilingual (English/Spanish preferred)
• Basic computer skills
• Child abuse Certification

High School Diploma/Equiv or better.
