What you’ll do

- Review medical history and evaluate each resident’s medical condition, functional abilities, limitations, restrictions, and rehabilitation potential.

- Assess residents’ ability to safely perform activities of daily living, including dressing, bathing, grooming, toileting, mobility, and other meaningful tasks.

- Establish and administer individualized occupational therapy treatment programs under the direction of the physician.

- Develop realistic, measurable, and achievable goals based on each resident’s capacity and tolerance.

- Provide direct treatment to improve function, independence, safety, and quality of life.

- Recommend, fit, and train residents on adaptive equipment, mobility devices, and techniques that support independence.

- Educate residents and families on therapy methods, home-safety strategies, and ways to support continued progress.

- Complete evaluations, weekly progress notes, recertifications, discharge documentation, and accurate clinical records in a timely manner.

- Monitor and document residents’ response to treatment and progress toward established goals.

- Collaborate with physicians, nursing staff, therapy colleagues, social services, and other disciplines to provide the best resident-centered care.

- Maintain compliance with confidentiality requirements, HIPAA, federal residents’ rights, and all applicable regulations.

- Perform other duties as assigned by the Director of Rehabilitation.

What you need

- Graduate of an accredited Occupational Therapy program.

- Current, active Indiana Occupational Therapist license and registration.

- Strong clinical judgment, communication skills, and a compassionate, patient-centered approach.

- Ability to work effectively with residents, families, and an interdisciplinary care team.

- Long-term care and/or geriatric care experience preferred but not required.
