Case Management Coordinator

Overview Randstad Healthcare is hiring for a Medical Case Manager in Cutler Bay. Candidates must reside in ZIP codes 33177, 33189, 33187, 33157, 33176, and 33186. Responsibilities As a Medical Case Manager, you will: Perform telephonic and face‑to‑face assessments, develop care plans, and coordinate all case‑management activities. Manage a caseload of supportive and medically complex members, ensuring their overall wellness. Determine appropriate services and supports, such as prior authorizations, PCP and skilled provider coordination, condition‑management information, medication review, and community resources. Develop proactive plans to address member issues and enhance short‑ and long‑term outcomes. Travel Requires 50‑75% travel within Miami‑Dade County for face‑to‑face visits at members’ homes, assisted living facilities, and skilled nursing facilities. Qualifications Bilingual in Spanish and English (fluent speaking and writing). Strong multitasking, prioritization, and adaptation skills in a fast‑paced environment. Excellent verbal and written communication skills. Ability to travel approximately 75% of the time within the specified ZIP codes.
