Service Coordination Manager

Service Coordination Manager The Service Coordination Manager (SCM) builds and fills shift schedules, ensures that the nurse’s skillset matches the clinical needs of the patient, facilitates communication between the branch office, patients and families, and caregivers (RN, LPN/LVN, HHA, CNA, etc.). The SCM conducts home visits to assess the environment and personalities of the patients and families, matches caregivers accordingly, and supports the hiring, onboarding and orientation process. The SCM also records time worked, obtains authorizations from insurance companies, and maintains strategic relationships with payroll, finance, human resources, leadership, clients, and nursing staff. Key Responsibilities Maintain a current patient roster (casebook) with all pertinent staffing information and monitor schedule changes. Conduct home visits to assess the environment and personalities of clients/families and match them with a caregiver. Perform telephone screens and face‑to‑face interviews with potential field clinician team members. Maintain recruitment tracking system(s). Prepare schedules in accordance with local, state or federal requirements and certification periods for clients and field staff. Co‑ordinate services provided to patients and control schedule changes. Call nurses daily from roster to fill open schedules and verify compliance of completed shifts. Assist in taking patient referrals and manage key performance indicators to enhance operational efficiency. Ensure field nurse hours are accurately recorded for payroll and conduct weekly client calls to keep families informed about schedule updates, gaps, or changes. Import authorizations received from RCM into Kantime (EMR system) and evaluate nearing end of patient authorization periods for renewal. Review nurse and patient records to ensure documentation is complete according to QAPI audit guidelines. Attend and complete mandatory training/in‑services as assigned, and adhere to company compliance and ethics expectations. Maintain patient and family confidentiality and maintain professional, supportive, and responsive communication skills. Perform other related duties as assigned. Qualifications High School Diploma or equivalent; 6 months or more of customer service experience. Cleared background screening, valid driver’s license with proof of insurance. Knowledge of Microsoft Office products. Clear written and verbal English communication skills. Associates degree (preferred) and healthcare experience (preferred). Experience organizing and managing office files, logbooks, and staff schedules. Strong communication skills and ability to manage a high volume of phone calls. Experience in a healthcare setting, especially home care or related fields. Ability to manage multiple projects and initiatives simultaneously. State–specific requirements (Florida, Illinois, etc.) as applicable. TB screening with 2‑step PPD or 1‑step PPD with proof within the last year (for certain states). Working Conditions / Equipment Use The position is predominantly desk‑based but involves significant telephone communication, occasional walking and light lifting of office records. Travel may be required for local or out‑of‑town seminars, conferences, or meetings. The role is stressful due to meeting deadlines. Pay Philosophy The starting salary is determined by skills, experience, education, certifications, and location. Compensation may include a bonus plan based on defined metrics.
