Organization VHVI OP Clarksville Job Summary Applies the nursing process to plan and implement patient care, including education and planning, and ensures effective outcomes while communicating pertinent information to the health care team. Position Hours Monday–Friday, Day shift; no nights, weekends, or holidays. Key Responsibilities Collaborate in establishing the plan of care and coordinate and implement care delivery while anticipating patient needs across the continuum. Analyze comprehensive information related to patient care and management to determine nursing diagnoses. Continuously utilize the nursing process to evaluate patient progress toward goals and apply appropriate interventions. Employ strategies to promote health and maintain a safe environment. Additional duties may be assigned as needed. Technical Capabilities Care Planning (Novice) Anticipate resource needs, set priorities and schedule activities. Handle multiple assignments simultaneously and adapt to changes with minimal disruption. Ensure issues are resolved promptly. Nursing Patient Education (Novice) Provide patient education in practical applications of moderate difficulty. Nursing Patient Assessment & Evaluation (Novice) Conduct primary care patient interviews and physical examinations. Recognize problems, ask relevant questions, and contact primary care providers and team members. Gather pertinent clinical data for referral and treatment pathways. Evidence-Based Practice (Novice) Search for evidence to answer clinical questions and work under the direction of others. Core Accountabilities Execute responsibilities with understanding of impact on the organization and team. Analyze moderately complex problems using technical experience and judgment. Provide informal guidance and support to team members. Communicate effectively and adapt style to various audiences. Deliver excellent services with compassion and problem solving. Position Qualifications Certifications : LIC-Registered Nurse (and other applicable licensure) Experience : Relevant experience; minimum 1 year (Graduate of an approved discipline‑specific program) Education : Graduate of an approved program.
