Since that time, our range of services has expanded in both scope and size to include a broad spectrum of in-home healthcare services across the nation.Duties & Skills:Conduct nursing assessments and develop individualized plans of care based on client needs.Monitor, identify, and document changes in clients' physical, emotional, and overall health.Administer medications and treatments as ordered and within scope of practice.Maintain accurate, timely, and confidential clinical documentation.Educate clients and caregivers on health conditions, medications, treatments, and disease management.Identify health and safety concerns within the home environment and respond appropriately.Collaborate with healthcare providers, caregivers, and other members of the care team to support coordinated care.Demonstrate strong clinical judgment, communication, organization, and time-management skills.Work independently in a home care setting while following professional nursing standards, organizational policies, and standards of care.We are seeking a compassionate and skilled Home Care Nurse to join our team.
