Hospital Social Worker II — Crisis Counseling & Discharge

Prime Healthcare Saint Francis Hospital in Evanston, IL is seeking a Social Worker for a full-time days schedule (8:00 a.m.–4:30 p.m.) with two weekend shifts per month. You will provide emotional support, crisis intervention, and psychosocial assessments for pediatrics through older adults and coordinate discharge planning.
