Job Responsibilities

• Assess patients in either inpatient or outpatient settings and formulate tailored treatment plans
• Implement physical therapy techniques focusing on enhancing mobility, strength, balance, and overall function
• Partner with nurses, physicians, case managers, and other rehabilitation professionals to ensure optimal patient outcomes
• Educate patients and their families on therapeutic objectives, discharge processes, and prescribe home exercises
• Document evaluations, treatments, and patient advancements adhering to facility and regulatory standards
• Engage in care team discussions and interdisciplinary conferences to promote seamless care delivery

Qualifications

• Doctorate in Physical Therapy (DPT) or equivalent from an accredited institution

Certifications

• Valid state licensure as a Physical Therapist
• CPR certification preferred
• Additional specialized certifications (e.g., in geriatric or neurological therapy) are advantageous but not mandatory
