Position Summary

The Hospice RN Case Manager provides skilled nursing care to hospice patients in their homes and serves as the primary case manager for assigned patients and families.

Working closely with the Hospice Interdisciplinary Team (IDT), this nurse develops, implements, and revises individualized plans of care to address patient needs, manage symptoms, promote comfort, and support families throughout the end-of-life journey.

-> Key Responsibilities

• Manage an assigned hospice caseload and coordinate care with the interdisciplinary team.
• Conduct comprehensive patient assessments and develop individualized plans of care.
• Provide pain and symptom management and other skilled nursing interventions.
• Perform clinical procedures, including medication management, wound care, catheter care, IV therapy, and diabetic care, as appropriate.
• Educate patients and families regarding diagnoses, medications, treatments, and care plans.
• Maintain accurate, timely clinical documentation in accordance with applicable regulations and agency policies.
• Participate in interdisciplinary team meetings, care conferences, and quality improvement activities.
• Supervise and support Hospice/Home Health Aides, including appropriate delegation and competency oversight.
• Participate in rotating on-call coverage to support patients and families when urgent needs arise.
• Assist with home health evaluations, care planning, physician coordination, and discharge planning as needed.

-> Required Qualifications

Education and Licensure

• Graduate of an accredited School of Nursing.
• Active, unrestricted Oregon Registered Nurse (RN) license.
• Current BLS certification or ability to obtain certification before hire.

Experience

• At least one year of nursing experience in hospice, home health, skilled nursing, or a related healthcare setting.
• Three or more years of relevant nursing experience preferred.

-> Knowledge and Skills

• Knowledge of hospice philosophy and end-of-life care practices.
• Understanding of applicable home health and hospice regulations.
• Experience with pain and symptom management.
• Knowledge of nursing procedures, medical terminology, and medication administration.
• Proficiency in clinical documentation and electronic medical records.
• Ability to work independently and manage competing priorities.
• Strong communication, patient education, and interpersonal skills.
• A compassionate, empathetic, and patient-centered approach to care.

-> Additional Requirements

• Valid Oregon driver's license.
• Reliable transportation and current automobile insurance.
• Clean driving record.
• Ability to travel regularly between Brookings, Gold Beach, Bandon, and patients’ homes.

-> Physical and Work Environment Requirements

This role involves home-based patient visits and community travel. The RN must be able to stand, walk, bend, kneel, stoop, climb stairs, and assist with moving patients, equipment, and supplies as needed.

The position also requires the ability to work in varied home environments, manage emotionally challenging situations professionally, and follow appropriate precautions when exposed to blood, bodily fluids, infectious materials, pets, tobacco smoke, and other environmental conditions.

Occasional weekend, holiday, and rotating on-call coverage is required.
