Hospice RN Case Manager

Location: Brookings, OR
Job Type: Full-Time
Compensation: Open To Discuss (DOE)

Position Summary

We are seeking a compassionate Registered Nurse (RN) to provide skilled hospice care, manage patient caseloads, coordinate individualized care plans, and support patients and families through end-of-life care.

Responsibilities

• Assess patients and develop individualized care plans.
• Provide pain and symptom management, medication administration, wound care, and other skilled nursing services.
• Educate patients and families on treatments and medications.
• Maintain accurate clinical documentation and regulatory compliance.
• Coordinate care with the interdisciplinary team and supervise hospice/home health aides.
• Participate in rotating on-call coverage.
• Travel to patient homes in Brookings, Gold Beach, Bandon, and surrounding areas.
  

Requirements

• Graduate of an accredited nursing program.
• Active, unrestricted Oregon RN license.
• Current BLS certification or ability to obtain before hire.
• Minimum 1 year of nursing experience in hospice, home health, skilled nursing, or a related setting.
• Valid Oregon driver's license, reliable transportation, current auto insurance, and clean driving record.
• Strong clinical, communication, organizational, and patient care skills.

Preferred: 3+ years of nursing experience and prior hospice or home health experience.
