Registered Nurse (RN) - Case Management

Coastal Home Health and Hospice

Travel RN Case Manager – Home Health

Schedule: Monday–Friday, 8 AM–5 PM

Assignment Details: Contract length: 13 or 26 weeks. Guaranteed hours: 40/week. Approximately 2–7 patient visits per day, primarily covering a specific geographic area with an average travel distance of approximately 60 miles one way. Documentation time included in the workday.

Requirements: Active Oregon RN license is required. Previous Home Health experience is required. OASIS experience is required. Experience with EMR system: Axxess.

Responsibilities: Responsible for start-of-care visits, recertifications, and discharges. Staff supervision and coordination. Home Health focus only, no Hospice coverage.
