Role Description This is a full-time, on-site Speech Language Pathologist role based in Sacramento, CA. The Speech Language Pathologist will assess, diagnose, and treat individuals with speech, language, cognitive-communication, and swallowing disorders in a home health setting. Day-to-day responsibilities include conducting comprehensive evaluations, developing and implementing individualized treatment plans, and documenting progress in accordance with clinical and regulatory standards. The role involves collaborating closely with patients, families, caregivers, and an interdisciplinary team to support functional communication and safe swallowing. The Speech Language Pathologist will also educate patients and caregivers on home exercise programs, strategies, and adaptive techniques to promote independence and improve quality of life. Qualifications Demonstrated proficiency in Speech Therapy, including assessment, treatment planning, and intervention. Experience working with Communication Disorders and Language Disorders across diverse populations. Knowledge and clinical experience in evaluating and treating Dysphagia. Master’s degree in Speech-Language Pathology from an accredited program. Current state licensure (or eligibility) as a Speech-Language Pathologist in California. Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) preferred or in progress. Strong documentation, time management, and case management skills in a clinical or home health setting. Ability to work collaboratively in an interdisciplinary team and communicate effectively with patients, families, and colleagues. Current CPR certification and valid driver’s license with reliable transportation for home visits. #J-18808-Ljbffr
