Reentry Health Case Manager

Via Care Community Health Center

Viacare Community Health Center in Monterey Park is seeking a Justice-Involved Case Manager who leverages personal lived experience of incarceration to support patients transitioning from jail or prison. This role involves advocating for patients, assisting with appointments, and maintaining accurate medical records. The ideal candidate has a Case Management certificate, a minimum of one year of medical experience, and is passionate about supporting the justice-involved community.
