POSITION SUMMARY
RN Case ManagerPOSITION DUTIES
Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient’s medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.MINIMUM REQUIRED QUALIFICATIONS
ME/compact RN license BLS 2+ years recent case management experience in an acute hospital setting Skilled experience in discharge planning and care coordinationPREFERRED QUALIFICATIONS
5+ years recent case management experience in an acute hospital settingLENGTH OF ASSIGNMENT
13 weeksSHIFT / HOURS PER WEEK
M-F 7:30a-4p plus every third weekendSYSTEMS
