Transform home-based care. As Circadia's full-time field home health NP, you'll see patients directly in their homes, act on predictive health signals in real time, and coach care teams to turn data into earlier interventions—reducing preventable hospitalizations. You'll blend direct patient care with tech-to-bedside integration through hands-on training, live demonstrations, and shadowed assessments within the home health setting.
Direct patient care: initiate/adjust plans of care; coordinate with referring and primary care providers.
Introduce the Circadia Device during in-home assessment and help set-up in a 2 minute process.
Data into action: Interpret trend-based insights from the Circadia platform, prioritize high-risk patients, and drive timely interventions to prevent decline and unnecessary ED visits.
Efficiently complete 6+ in-home visits per day across designated home health patients in the SoCal/OC area, focused on new admissions
Quality & product feedback: Surface field insights to improve workflows and tech integration in a home health setting
Cover sites within a 30 mile radius; for sites outside of that radius, mileage and lodging covered per policy.
Visits: Target 6+ patient home visits/day
Adoption & impact: trend toward fewer falls/hospitalizations and improved home health star ratings over time.
Certified and licensed Nurse Practitioner in CA; ANCC (or equivalent) in Adult/Family/Geriatric/Acute.

• 2+ years NP experience with complex adult populations in home health or post-acute settings; Comfortable conducting face-to-face visits in a home environment, including with patients requiring complex care.
• English required.

Google Suite, multiple EMRs (HCHB experience a plus), Slack, AI transcription/tools.
Spanish preferred.
