Key Responsibilities:
Provide management and delivery of patient care plans in facilities and communities
Collaborate with the care team
Support patients in Mechanicsburg, PA and surrounding areas

We are looking for compassionate nurse practitioners with:
Bachelor of Science (BSN) degree from an accredited School of Nursing
Master of Science (MSN) degree from an accredited School of Nursing
A minimum of five (5) years of clinical experience
Hold, in good standing, a license as a Registered Nurse and Nurse Practitioner issued by the state in which you work
Experience in a community based Home Health or Hospice/Palliative Care practice is strongly preferred.
Qualifications:

• Master's degree or higher
• Clinical Experience (2+ years)

~1+ years of experience in ANY of the following:

• Hospice
• Palliative Care
• Home Health

ALL of the following valid licenses/certifications:
Nurse Practitioner (NP) in Pennsylvania (PA)
Registered Nurse (RN) in Pennsylvania (PA)

ANY of the following valid licenses/certifications:
American Heart Association Basic Life Support (AHA BLS)
American Heart Association Advanced Cardiac Life Support (AHA ACLS)
