Patient-facing primary care provider

The Advanced Practice Provider delivers compassionate, comprehensive, and personalized primary care to patients across the lifespan.

Working as a PA-C or NP within applicable licensure, certification, credentialing, and practice requirements, the Provider evaluates patients, establishes diagnoses, develops and manages treatment plans, prescribes therapies, performs appropriate office procedures, and coordinates care with physicians and the broader clinical team. The role supports Allendale Family Practice’s commitment to trusted relationships, preventive care, open communication, and high-quality service to individuals and families in the Allendale community.

Founded in 2004, Allendale Family Practice is an independent, patient-focused family medicine practice serving patients from newborns through older adulthood. AFP provides preventive and acute care, pediatrics, geriatric care, chronic-condition management, women’s health, mental-health support, immunizations, minor procedures, and coordinated medication and care-management services. Our mission is to provide compassionate, comprehensive, and personalized healthcare while empowering patients to achieve optimal well-being.

Provide evidence-based primary care for patients across the lifespan, including preventive, wellness, acute, follow-up, and chronic-care visits.

Obtain and review medical histories; Diagnose and treat acute illnesses, injuries, and chronic conditions within the Provider’s education, competency, privileges, and legal scope of practice.

Develop, document, and manage individualized treatment plans, including medications, non-pharmacologic therapies, referrals, follow-up care, and patient self-management goals.

Prescribe and manage medications responsibly, including medication reconciliation, refill review, monitoring for effectiveness and adverse effects, and compliance with controlled-substance requirements.

Perform or assist with appropriate ambulatory procedures, which may include wound care, laceration repair, minor skin procedures, joint injections, immunizations, and other services for which the Provider is trained and privileged.

Provide health promotion, risk-reduction counseling, disease-prevention education, and shared decision-making that respects each patient’s goals, values, and preferences.

CARE COORDINATION, DOCUMENTATION & TEAM RESPONSIBILITIES

Maintain timely, accurate, complete, and clinically appropriate documentation in the electronic health record, including diagnoses, orders, medications, coding support, patient instructions, and follow-up plans.

Manage assigned inbox, portal, refill, test-result, referral, prior-authorization, and clinical-document workflows within established practice standards.

Collaborate with physicians, other Providers, medical assistants, nurses, pharmacy/care-management resources, administrative staff, specialists, hospitals, and community resources to support continuity of care.

Recognize clinical limits and elevate, consult, refer, or transfer care when patient needs exceed the Provider’s scope, competency, available resources, or outpatient setting.

Participate in quality improvement, population-health initiatives, preventive-care gap closure, patient-safety activities, peer review, and compliance efforts.

Maintain confidentiality and comply with HIPAA, OSHA, infection-control standards, payer requirements, clinical protocols, and all applicable federal and Michigan laws and regulations.

Graduate of an accredited Physician Assistant or Nurse Practitioner program; master’s degree or higher as required for professional credentialing.

Current prescriptive authority and eligibility for DEA registration when required by assigned duties; ability to meet all payer enrollment, credentialing, and privileging requirements.

Clinical preparation and certification appropriate to the patient population served; family medicine or primary-care experience preferred.

Proficiency with electronic health records and electronic communication; experience with athenaOne or a comparable ambulatory EHR is preferred.

Ability to perform the essential functions of an ambulatory clinical position, including prolonged sitting, standing, and walking; frequent computer and telephone use; use of standard medical equipment; The Provider may be exposed to infectious materials and must follow required safety precautions.

Duties may be modified based on patient needs, practice operations, credentialing, scope of practice, and applicable law. Employment remains subject to AFP policies and any governing employment agreement.
