Care Plus Hospice Care LLC is seeking a compassionate and dedicated After Hours Hospice Nurse (On-Call) to join our team in the Phoenix AZ valley. This is a meaningful opportunity to provide expert clinical care and emotional support to patients and families during their most critical moments — after hours when they need it most. If you're a skilled nurse with a heart for end-of-life care, we want to hear from you.
Conduct skilled nursing assessments and interventions for hospice patients in home or facility settings
Manage and adjust pain and symptom control measures in accordance with the patient’s care plan
Communicate effectively with the on‑call physician and interdisciplinary team regarding patient status changes
Document all clinical encounters, interventions, and patient updates accurately and in a timely manner
Coordinate after‑hours care needs including medication changes, equipment, and emergency support
Active RN or LPN license in the state of Arizona (RN preferred)
Minimum 1‑2 years of hospice, palliative care, or home health nursing experience
Ability to work on‑call shifts including evenings, weekends, and holidays
Proficiency in electronic medical records (EMR) documentation
