Physical Therapist / Therapy (PT) - Home Health Care

Deliver exceptional patient care while exploring a dynamic home health therapy opportunity supporting adults with a variety of chronic and post-operative diagnoses. This contract role focuses on hospital-based therapy services where you will travel throughout multiple surrounding areas, managing your own patient schedule and ensuring continuity of care in a supportive, collaborative department culture.

No weekend rotation
No major holiday coverage
Minimum of 1 year experience in home health therapy (PT required)
Active state licensure for Physical Therapy
Comfort with adult patients managing chronic diseases, orthopedic injuries, and post-surgical recovery
Willingness and ability to travel approximately 60-80 miles per day; mileage reimbursement provided
Seasoned home health therapists welcomed, including first-time travelers with relevant experience
Provide comprehensive patient assessments and therapy services in various home settings
Maintain accurate electronic documentation using EPIC Rover, Focura, and Home Care Home Base on company-provided Samsung Note 9 device
Adhere to department requirements: scrubs in grey, free on-site parking available
Structured orientation and onboarding for facility and department processes
