RN Home Health Care

CarePartners is seeking a compassionate and experienced Registered Nurse for our Home Health Franklin NC Team within our Healthcare & Medical Services department. As an RN in this role, you will provide in‑home nursing care to patients in the Franklin, NC community, ensuring high‑quality, patient‑centered services that align with CarePartners’ mission to improve community health. You will conduct comprehensive assessments, develop and update individualized care plans, administer medications and treatments, and provide education to patients and their families on disease management, medication adherence, and preventive care. Collaborating closely with physicians, therapists, social workers, and other members of the healthcare team, you will coordinate services to support safe transitions of care and reduce avoidable hospitalizations. You will document all care in accordance with regulatory and organizational standards, use electronic health records to track outcomes, and communicate regularly with providers in our partnered network to support efficient and coordinated healthcare service delivery. This role requires strong clinical judgment, excellent communication skills, and the ability to work independently in the field while demonstrating compassion, cultural sensitivity, and respect for patient autonomy.
