Ancillary Staff per Provider: multiple
Number of Exam Rooms per provider: 3
Medical Imaging
Lab and Pharmacy
Services/Procedures offered in the practice: X-rays, MRIs, CT scans, Nuc Med and Ultrasounds, Fluoroscopy studies include Swallow, BE, Myelogram, LP, no mammo reads or breast biopsies will be included.
Volume expectations (these numbers are on the high end): Xray - 30, CT - 30, NM - 3, MR - 7, US - 10
Board Certified - [REQUIRED]
Active IA License or Active IMLC LOQ - [REQUIRED]
Fellowship Trained (General Rad with Mammography and Fluoroscopy) - [REQUIRED]
Clean Malpractice/Background - [REQUIRED]
