Registered Nurse (RN) - Hospice
As a Hospice RN, you provide end-of-life care focused on comfort, symptom management, and supportive documentation for patients and families.
You work within interdisciplinary teams to honor patient goals and coordinate with community resources for home or facility-based care.
Assess symptoms, manage pain and comfort, and provide comprehensive nursing care
Coordinate with physicians, social workers, chaplains, and home health aides
Educate families about advance directives, care planning, and bereavement support
Document care plans and patient status in the EHR; Support discharge planning and transitions to home or hospice facilities
ADN acceptable with hospice or palliative care experience
Current, active Registered Nurse license in the United States
Palliative Care certification such as CHPN preferred but not required
Experience in hospice or palliative care; strong communication, assessment, and family-support skills
Experience providing end-of-life nursing care in home or facility settings
Experience coordinating with interdisciplinary teams and community resources
Experience documenting care plans and patient goals in the EHR
CHPN or equivalent hospice certification
Experience with care transitions and bereavement support
Hospice teams provide care in patients' homes or hospice facilities; Shifts may include days with on-call coverage; documentation and phone triage are common components
