This role involves direct clinical care, timely documentation, clear communication, and coordination with clients, families, and clinical leadership to ensure high-quality service delivery.Primary ResponsibilitiesField Case ManagementAssess mental, physical, and emotional status, creating plans of treatment and coordinating care, collaborating with the Nurse Supervisor or Manager of Clinical Practice.Documentation and CommunicationReport changes in client medical status and emergencies promptly to the Nurse Supervisor or Clinical Manager.Complete required clinical documentation accurately and on time using the electronic medical record system, ensuring compliance with regulatory and organizational standards.Communicate schedule changes such as tardiness, vacation, requested time off, change of status, or reduction in hours to Care Coordinators promptly to support appropriate coverage.Confirm weekly schedules with the Care Coordinator to ensure required staffing levels are maintained.QualificationsCurrent, active RN license in the state of practice, in good standing.2 years of home health experience preferred.Proven ability to work collaboratively as part of a healthcare team.Commitment to patient centered, safe, and compassionate care.Work Environment • Patient care is delivered in diverse settings, including private homes, assisted living facilities, and clinical environments. • Daily travel to patient locations is required. • Routine exposure to blood, body fluids, and other potentially infectious materials.
