We are a well-established Homecare practice committed to providing high-quality, personalized care to our patients. Our team focuses on restoring mobility, improving strength, and providing exceptional patient care in a supportive environment.
Conduct comprehensive physical therapy evaluations and create personalized treatment plans.
Provide hands-on physical therapy and exercise programs to patients of all ages.
Document patient progress and treatment plans in compliance with state regulations.
Collaborate with other healthcare professionals to provide the best care possible.
Licensed Physical Therapist (PT) in the state of New York.
Doctor of Physical Therapy (DPT) Degree from an accredited institution.
Previous experience in outpatient care or a rehabilitation setting is preferred, but not required.
Required Licenses and Documents for New York Physical Therapists:
Physical Therapy License issued by the New York State Education Department (NYSED).
Proof of passing the National Physical Therapy Exam (NPTE) .
CPR Certification (Adult/Infant/Child).
