Registered Nurse - Home Health & Hospice - Evening Shift

Hospice / Palliative Care RN – Home Health/Hospice & Skilled Nursing

Shift: 8-hour days, 08:00–17:00 (40 hours can be offered). Call: On-call rotations (Fri–Sun 20:00–08:00); outpatient nurses rotate 2 evening on-call shifts (Sat & Sun 20:00–08:00) every ~4–5 weeks.

Contract Length: 13 weeks. Guaranteed Hours: 40 hours per week.

Minimum 2 years RN experience required. Required certification: BLS. Required work-setting experience: Home Health/Hospice and Skilled Nursing. EMR experience: EPIC Remote Client.

Provide end-of-life care and symptom management for geriatric patients. Conduct intake assessments and set goals collaboratively with patients and families. Manage pain and medication, including breakthrough dosing and pharmacologic/non-pharmacologic interventions. Educate patients and families on care processes, Medicare regulations, and preventive measures. Ensure alignment with advanced directives and ethical issues surrounding end-of-life care.

Orientation: 40 hours plus standard facility orientation. Scrubs: Navy blue; not provided. COVID-19 vaccine not required; Flu vaccine required (medical/religious exemptions only).
