Description

Conduct thorough evaluations for patients in both inpatient and outpatient environments and craft tailored treatment strategies
Implement therapeutic interventions that focus on mobility, strength, balance, and overall functional improvement
Work cohesively with nurses, physicians, case managers, and other rehabilitation professionals to deliver optimal patient outcomes
Guide patients and their families through therapeutic objectives, discharge planning, and necessary home exercises
Accurately document evaluations, treatments, and patient progress in accordance with facility expectations and regulatory standards
Engage in multidisciplinary team rounds and interdisciplinary conferences to ensure comprehensive care
Education
~ Doctorate in Physical Therapy (DPT) or an equivalent degree from an accredited institution

Certifications
State licensure as a Physical Therapist
CPR certification is preferred
Additional specialized certifications (e.g., neurological, geriatric) are an advantage, though not essential
Skills
Prior experience in a similar role is highly desirable
