Summary We are seeking a compassionate and skilled Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team at Candlewood Healthcare & Rehabilitation. In this role, you will provide high-quality nursing care to our residents, ensuring their health and well-being while contributing to our mission of family-focused care.

The typical schedule for this opportunity is Three (3) 8-hour shifts a week. The standard schedule for this position includes an every other weekend rotation. This position is available for 1st Shift from 7am - 3pm.

Responsibilities Administer medications and treatments as prescribed by physicians.
Monitor patient health and provide updates to the healthcare team.
Assist in the development and implementation of individualized care plans.
Perform assessments, including vital signs and medical history evaluations.
Educate patients and families on health management and wellness practices.
Coordinate with interdisciplinary teams to ensure comprehensive care delivery.
Document patient progress accurately in electronic health records.
