In this role, you will work directly with clients to evaluate, develop, and implement individualized treatment plans addressing speech, language, cognitive-communication, and swallowing disorders. Additionally, you will collaborate with interdisciplinary healthcare teams to ensure seamless care coordination and promote optimal patient outcomes.

Conduct comprehensive speech and language evaluations to determine patient needs.

Develop and implement individualized treatment plans in accordance with physician orders and agency standards.

Provide therapeutic interventions targeting speech, language, voice, cognitive-communication, and swallowing disorders.

Assess and document patient progress, modifying treatment plans as clinically appropriate.

Collaborate with interdisciplinary healthcare teams to ensure continuity of care and optimal outcomes.

Communicate effectively with clients, families, caregivers, and physicians.

Maintain compliance with all legal, ethical, and professional standards.
