Physical Therapist /Occupational Therapist - Home Health Care

Pride Health is hiring a Occupational Therapist (OT) - Home Health to support our client's healthcare facility in Easton, MD.

Monday-Friday, 8:00 AM - 4:30 PM (a { text-decoration: none; Job Type: Local contract
Contract Length: 14 Weeks
Mileage Reimbursement Rate (if applicable) : $0.51/ mile
*Pay offered is based on experience, expertise, credentialing, and education.

Perform comprehensive Occupational Therapy evaluations and OASIS-driven assessments in the home setting.
Develop, implement, and update individualized Plans of Care (POC) based on physician orders and patient-specific needs.
Recommend adaptive equipment, environmental modifications, and therapeutic interventions to maximize patient safety and independence.
Collaborate closely with nurses, physical therapists, speech therapists, physicians, case managers, and other interdisciplinary team members.
Communicate effectively with patients, caregivers, providers, and office staff to coordinate high-quality patient care.
Meet established productivity standards while delivering exceptional patient-centered care.
Active Maryland Occupational Therapist License in good standing.
Previous Home Health experience required .
