Occupational Therapist (OT) will provide coordinated care to clients of all age groups. Plans implements and evaluates client care plans to restore or maintain patient well-being. Evaluates occupational roles and performance of clients with physical, psycho-social, and/or cognitive impairments. Document, prepare/revises, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis.

Apply concepts of infection control and universal precaution in coordination/performing client cares activities to protect both client and staff (OSHA).

Assess and facilitate through direct therapeutic intervention clients' ROM, strength, coordination need for and use of adaptive equipment and environmental modifications, functional mobility, and safety issues as related to clients' ADL performance needs.

Provides effective communication to clients, their families, team members, and other health care professionals, and regular participation in Case conferences/Team Meetings. Evaluate client's occupational performance in the areas of self-care, work, leisure, and rest. Completes appropriate documentation in a timely manner to assure compliance with company policy.

Preferred, Home health experience.

Home Health OT Requirements: Therapist skills as defined as generally accepted standards of practice. Excellent observational and problem-solving, decision-making and report writing skills. Bachelor of Science in Occupational Therapy from an American Physical Therapy Association approved program.
