LPN Home Care. Make Your Own Schedule

LVN will provide direct patient care to home bound patients.

Responsibilities and Duties: Plano, Richardson, Sachse Area

Provides wound care, disease process education, medication administration, and other clinical duties as prescribed by NP's, PA's, and Physicians.

Carries out the clinical plans of care to assist patients maintain their independence.

Coordinates with the interdisciplinary clinical team regarding patient changes in condition and proactively communicates patient's clinical needs.

Provides education to patients and families on proper home health care procedures and strategies.

Advocates for appropriate patient care and recommends strategies to help improve the patient's quality of life.

Qualifications and Skills

Current Texas LVN License.

Current Certification in BLS/CPR

Valid Texas Driver's License and Automobile Insurance

Knowledge of WellSky EMR software is a plus
