LCSW- Mental Health Clinic

LifeLong Medical Care is searching for a Licensed Clinical Social Worker (LCSW), Licensed Professional Clinical Counselor (LPCC), to join the team at our Over 60 Health Center . Under the general supervision of the Associate Behavioral Health Director, the LCSW/LPCC will work with an inter-disciplinary team to ensure that quality psychosocial services are delivered to health center patients.

As a Federally Qualified Health Center in Northern California with over 50 years of dedicated service to the community, LifeLong serves over 66,000 patients in the East Bay Area's socio-economically underserved regions. We provide medical, dental, behavioral health, and school-based services at more than 15 primary care sites across Oakland, Berkeley, and West Contra Costa County. From opening a residential respite program for homeless persons discharged from hospital to offering free testing to the community, we are innovative and responsive to our times' social and medical needs. We offer a competitive annual salary of $101,753-$122,928 for LCSW/LMFT/LPCC , depending on years since licensed, and excellent benefits: medical, dental, vision, Flexible Spending Accounts, dependent and domestic partner coverage, 403(b) retirement savings plan, and loan repayment programs. LifeLong is a qualified site for federal and state loan repayment programs (HPSA score is 16). Provides comprehensive psychosocial assessments, mental health diagnoses, treatment plans and implements short-term counseling using appropriate modalities for patients with issues such as: depression, anxiety, trauma, grief/loss, addiction, domestic violence, behavioral health problems, and psychosocial stressors. Works predominantly within clinic, but may include home visit services.

Provides short-term case management services for patients participating in treatment.

Crisis intervention includes assessment, de-escalation, and notifying the public mental health crisis response team if necessary.

Works with team members to provide follow-up assessment and prevention planning after any crisis, including follow-up with outside agencies.

Maintains clinical and administrative records in accordance with contractual and billing requirements and submits encounter forms on a weekly basis.

promotes integrated primary/mental health care in coordination with medical providers and other inter-disciplinary team members.

Participates in quality improvement efforts related to behavioral health and integrated models of care.

May serve as supervisor for psychology or MSW interns and may provide ASW or AMFT or APCC clinical supervision.
