Physical Therapist (PT) - PRN- Home Health Care

$50 - $60 per hour

Job Title: PRN Physical Therapist - Home Health
Location: Based in Columbus, OH (Travel required across regions, including Mahoning County)
Employment Type: PRN (As Needed)
Pay Range: $50 - $60 per hour (depending on experience)

Position Summary
We are seeking a licensed and motivated PRN Physical Therapist to join our growing home health team. This position is ideal for therapists who value autonomy, flexibility, and one-on-one patient care in a home setting. You will conduct evaluations and provide skilled physical therapy to patients in their homes throughout Columbus and extended areas, including Mahoning County .

Responsibilities

Perform in-home physical therapy evaluations and establish individualized care plans

Deliver skilled therapy to improve strength, mobility, function, and quality of life

Educate patients and caregivers on therapy techniques and safety

Accurately document all services provided in a timely manner using electronic medical records (EMR)

Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated patient care

Travel to patient homes within assigned service area, including flexibility to cover visits in Mahoning County

Requirements

Current Ohio Physical Therapist (PT) license in good standing

Minimum of one (1) year of experience in a clinical or home health setting preferred

Reliable transportation and willingness to travel extended distances as needed

Valid driver’s license and auto insurance

Strong documentation and time management skills

Comfortable working independently in patients’ homes

Preferred

Experience with EMR systems (e.g., WellSky, Homecare Homebase, Kinnser)

Familiarity with Medicare and OASIS documentation standards

Prior home health or geriatric care experience
