Group Home Direct Support Professional Lead

Position Summary
Home Sweet Home Care is seeking a compassionate, dependable, and experienced Direct Support Professional (DSP) Team Lead to provide leadership and direct support services at our Taylor Street Group Home in Fredericksburg, Virginia.

The DSP Team Lead serves as the on‑site leader responsible for ensuring high‑quality person‑centered services, regulatory compliance, staff accountability, and the health, safety, and well‑being of individuals receiving services.

This position combines hands‑on direct support responsibilities with leadership duties and serves as the primary liaison between residential staff and agency leadership.

Essential Responsibilities
Resident Care & Support

Provide direct support services to individuals with intellectual and developmental disabilities.

Assist individuals with activities of daily living (ADLs).

Support community integration and participation.

Assist individuals in achieving ISP goals and outcomes.

Promote independence, dignity, choice, and self‑determination.

Leadership & Oversight

Lead assigned shifts and provide direction to DSP staff.

Monitor staff performance and provide coaching and support.

Assist with scheduling, shift coverage, and staff accountability.

Ensure all required services are delivered according to ISP requirements.

Report concerns, incidents, and compliance issues to agency leadership.

Assist with onboarding and mentoring new staff.

Documentation & Compliance

Review daily progress notes for accuracy, completeness, and timeliness.

Ensure documentation reflects ISP goals, outcomes, community participation, and person‑centered supports.

Monitor compliance with DBHDS Regulations, HCBS Requirements, Human Rights Regulations, Risk Management Procedures, Agency Policies and Procedures, Safety & Risk Management.

Conduct routine health, safety, and environmental inspections.

Ensure medication administration and observation procedures are followed.

Monitor for behavioral, medical, and environmental risks.

Complete incident reports and follow‑up documentation as required.

Support emergency preparedness and crisis response efforts.

Community Integration

Support individuals in participating in community activities and events.

Encourage engagement with people outside the residence.

Promote individualized, non‑large group activities based on personal interests and goals.

Support individuals in building natural supports and meaningful relationships within the community.

Minimum Qualifications
Required

High School Diploma or GED.

Minimum of two (2) years of experience supporting individuals with intellectual and developmental disabilities.

Valid Driver's License.

Acceptable Driving Record.

Ability to pass required background checks and screenings.

Ability to work independently while leading and supporting a team.

Preferred

DSP Certification.

CPR / First Aid Certification.

Experience with Group Home Operations, Sponsored Residential Services, DBHDS Programs, HCBS Compliance, Staff Supervision, Electronic Documentation Systems.
