Referral Coordinator
The Referral Coordinator participates as an integral member of the interdisciplinary care team at ANHC. Key roles are coordination of services for panel patients, focusing on those whose face difficulty in navigating the healthcare system, management of referrals, and identifying relevant community resources. The Referral Coordinator may collaborate with outside service providers and provide applicable information to ensure seamless transitions of care. The Referral Coordinator works within the context of a primary care medical home, from a patient centered team approach, and in continuous partnership with families and the health care team to promote timely access to needed care. Panel patients may vary by panel, and may include patients experiencing homelessness, pregnancy, HIV, mental health issues, chronic diseases, and/or social issues. The Referral Coordinator is expected to be willing and able to provide services for all types of patients with compassion.
Coordination of Care
Coordinates care for panel patients before visits, after visits, and between visits and may accompany the primary care provider during the visit, with special focus on patients at higher risk.
Manages referral process from start to finish, including follow up on incomplete referrals.
Coordinates with medical facilities, specialists, and hospitals to ensure continuity of care.
Coordinates care with outside agencies or case managers as needed to meet patient needs.
Provides coverage for other Referral Coordinators when necessary.
Reviews status of eligibility for ANHC or external financial programs and educates as necessary.
Follows up with high risk patients regarding missed referral appointments to identify barriers.
Assists with accessing patient care items such as procuring durable medical equipment when patient resources are limited.
Assists patients to access appropriate assistance programs including insurance, Medicaid, and ANHC programs such as Ryan White, Health Care for the Homeless or Sliding Fee discount.
Quality Improvement
Accurately uses appropriate software to track care, in order facilitate data collection and process improvement.
Attends appropriate monthly pod meetings and referral coordinator meetings.

Physical, Sensory, Environmental Qualifications:
Must be able to communicate clearly via telephone, verbally in person, and in writing. Must be able to use a computer, multi-line phone, and other office equipment
Work is performed in a healthcare clinic and office setting. On an intermittent basis remote work may be required. Education, Certification and Licensure: High School Diploma or GED.
