Dietician - Nutrition Clinic

Job Summary Member of the care team that supports providers in addressing patients’ health and wellness dietary‑related needs. Provides group and individual education on dietary guidelines, self‑management, and dietary resources under general supervision, and works collaboratively with other care providers to develop patient‑centered health promotion plans. Conducts dietary consultations and delivers coaching, consultation, and intervention to patients with chronic or acute dietary needs.
Key Responsibilities Build and maintain an active patient case load to meet productivity requirements.
Conduct nutrition consultations, advise patients on healthy nutrition practices, and track progress in regular follow‑ups.
Document all patient contacts within 24 hours using the EMR.
Develop, implement, and evaluate evidence‑based dietary education curricula for individual and group sessions.
Collaborate with providers for warm handoffs, referrals, disease registries, and self‑referral identification.
Create customized meal plans, grocery lists, and teaching materials.
Educate patients and families on dietary strategies, medication interactions, and self‑management techniques.
Monitor and intervene to overcome barriers to care and promote adherence to care plans.
Facilitate support groups and psycho‑education on Health, Nutrition, and Exercise.
Document all patient contacts promptly and complete assigned tasks within deadlines.
Participate in daily huddles, team meetings, supervisory sessions, community events, and continuing education programs.
Advocate for patient needs and act as liaison among communities, individuals, and other service providers.
Perform other duties as assigned.
Clinic‑wide Responsibilities Treat guests, patients, physicians, and other employees with care, courtesy, and respect.
Respond quickly and appropriately to customer requests and clinic communications.
Seek ways to better meet customer needs.
Work cooperatively within own department and other areas, accept additional responsibilities, and assist colleagues.
Continuously look for and suggest improvements; implement them as appropriate.
Keep appropriate people informed; communicate clearly, listen attentively, and maintain confidentiality.
Use personal cell phone for timekeeping system access.
Present a positive image of Western Wayne Family Health Centers, carry out assignments with minimal direction, maintain timeliness, confidentiality, and proper absence notification.
Work weekends and shifts when necessary.
Qualifications Bachelor’s degree in health education, health promotion, public health, social work, nursing, dietetics or a related field.
Registered Dietitian (RD) or Registered Dietitian Nutritionist (RDN).
One (1) year of experience in a public health setting or community‑based environment.
One (1) year of experience with an electronic medical record system.
Bilingual (English/Arabic or English/Spanish) preferred.
Proficiency with Microsoft Office Suite products.
Knowledge of dietary public health problems, principles, and practices, including basic epidemiology.
Understanding of multiple chronic diseases and ability to deliver targeted dietary recommendations.
Ability to adapt quickly to changing conditions and incorporate new processes.
Strong communication, teaching, and motivational interviewing skills.
Experience establishing and maintaining effective working relationships with patients, staff, and the public.
Ability to work in a multicultural setting and address cultural and socioeconomic barriers.
Excellent organizational and prioritization abilities.
Ability to design, prepare, deliver, and modify course curriculum and educational aids.
Knowledge and ability to use health‑care software and EMR system.
Adequate transportation to visit all WWFHC sites.
Adherence to Academy of Nutrition and Dietetics guidelines, organizational policies, and confidentiality/privacy requirements.
Availability to work flexible days and hours, including weekends and shifts as necessary.
