Physical Therapist (PT) - Hospital Rehab

We are a therapist-owned mobile neuro practice in the Triangle, NC areas treating adults diagnosed with stroke, Parkinson's, MS, TBI, SCI, vestibular disorders, and other neurologic conditions. 1:1 outpatient-style care in the patient's home. Medicare Part B, so no OASIS and no two-hour start-of-care paperwork.

This is a tight local territory. You are would be a W2 employee, not a 1099 contractor. Full health, dental, and vision insurance. Retirement with match. Company tablet and equipment.

You get 20+ neuro specialized CEUs a year.

Office hour rate: $50+

Target Total Salary: $82,000-$105,000 at full caseload (27-30 visits/week)

If you are in a hospital outpatient clinic: you are probably running 12-15 patients a day in 45-minute slots against a productivity target. If you are in corporate home health: you are drowning in OASIS, chasing visit counts, and watching insurance cut patients off exactly when they start making progress.

If you are doing gig or contract mobile PT: you are a 1099. You eat the self-employment tax. We pay per visit too, but you are a W2 employee with benefits and an established local caseload instead of a contractor chasing whatever the algorithm sends.

And a cash-pay wellness continuum so your patients can keep working with you after Part B ends, which means you can provide long-term outcomes instead of running an eight-week revolving door.

Deliver high-intensity, evidence-based neurologic rehab for adults with PD, stroke, MS, TBI, SCI, and vestibular conditions

Conduct home safety assessments and caregiver training

Active NC PT license in good standing (New grads with strong neuro interest are also welcome, we have mentorship structures)

Genuine commitment to neurologic and/or geriatric rehab
