We are seeking a Licensed Practical Nurse who is adaptable, detail-oriented, and confident in managing both routine and complex care situations within our patient centered medical home setting. The ideal candidate will work closely with our medical provider team to deliver high quality, accessible care to our diverse patient population. You will apply your clinical expertise to perform a variety of complex nursing tasks and duties utilizing the nursing process. Insurance - Medical, Dental, Vision, Life, and Disability

• Graduate of a school of nursing accredited by National League of Nursing.

Current license to practice as a Licensed Practical Nurse in the State of Ohio.

One to three years full-time experience in nursing practice is preferred.

Computer experience desirable.

Collects patient data through physical examination techniques, interview, and observation.

Documents all nursing care provided according to policy.

Formulates plan of care for clients under direction from providers.

Obtains referrals from other physicians and specialists.

Follows approved protocols/standing orders for client care.

Responds to crisis situations according to policy.

Utilizes client education for emotional and physical health maintenance.

Performs clinic and outreach related tasks.

Utilizes on-going data collection skills.

Participates in Quality Improvement activities.

Establishes and maintains professional working relationships with other members of the health care team.

Participates in planning and problem solving.

Maintains knowledge of, and complies with, personnel, nursing, medical, and clerical policies and procedures.

Assists with seasonal programs as needed.

Performs nursing visits.

Completes patient health screenings including hearing, vision, and all CLIA waived testing.

Maintains stocks of medicines and medical supplies as necessary.

Educates and advises patients on specified medical issues within established parameters.

Adheres to the standards and policies of the Organizational Privacy/Security and Compliance Programs, including the duty to comply with applicable laws and regulations (HIPAA, OSHA, OIG, guidelines, and other State and Federal laws). Those physical movements and the degree of mobility, manual dexterity, and hand-eye coordination commonly associated with duties in an office setting will be performed repetitively. The ability to distinguish letters and symbols and utilize telephones, computer terminals, fax machines, and copiers is required.
