Licensed Practical NurseProvides direct patient care and related support to medically fragile clients and their families under the supervision of a Registered Professional Nurse or supervising licensed therapist, and within the realm of accepted standards of practice for a Licensed Practical Nurse as stated in the Nurse Practice Act for the state of practice.Duties and ResponsibilitiesOngoing interdisciplinary assessment of the patientImplementation and evaluation of the plan of care in partnership with the patient, representative (if any) and caregiver(s)Providing services that are ordered by the physician as indicated in the plan of carePatient, caregiver, and family counselingPatient and caregiver educationPreparing clinical notesCommunication with all physicians involved in the plan of care and other health care practitioners (as appropriate) related to the current plan of careCoordination with known external agencies providing care and services to the same patientParticipation in the agency's QAPI programParticipation in agency-sponsored in-service trainingProvides assistance in the implementation of the plan of careAssuring that patient needs are continually assessedEnsures delivery of services is maintained in accordance with physician orders and with client consentObserves, records and reports reaction to treatment and any changes in patient condition to appropriate personnel and/or physicianFollows established departmental policies, procedures, and objectivesPractices safety, environmental, and/or infection control measuresPerforms miscellaneous job-related duties as assigned by Clinical Manager/AdministratorParticipation in regular on-call rotation as directed by the Administrator
