You will work closely with an interdisciplinary home health team, collaborating with nursing, occupational therapy, speech therapy, and medical social work to coordinate care, update plans of care, and support smooth transitions from hospital or facility to home. Clear, timely electronic documentation in the agency's EMR system is an essential part of the role, ensuring regulatory compliance and contributing to quality metrics and clinical outcomes. The schedule typically follows daytime hours with flexibility based on patient needs and visit volumes. Caseloads and daily visits are structured to allow for thorough assessments, interventions, patient and caregiver education, and travel time between homes. Patient acuity and visit expectations are designed to support safe, high-quality care rather than rushed encounters.
