Administer prescribed medications and treatments accurately and in accordance with established policies and procedures

Monitor and document resident vital signs, health changes, and responses to treatment on an ongoing basis

Perform wound care, catheter care, and other skilled nursing procedures as directed by the care plan

Communicate resident status updates clearly and promptly to the supervising registered nurse and attending physician

Assist in developing, reviewing, and updating individualized resident care plans in collaboration with the interdisciplinary team

Respond promptly and calmly to resident needs, emergencies, and changes in condition

Provide emotional support and education to residents and their family members

Supervise and delegate appropriate tasks to certified nursing assistants while ensuring quality care delivery

Maintain accurate, timely, and thorough documentation in electronic health records

Follow all infection control, safety, and regulatory compliance standards

Current and valid Connecticut LPN license in good standing

Graduation from an accredited Licensed Practical Nurse program

Minimum of one year of clinical nursing experience, preferably in a long-term care, skilled nursing, or assisted living environment

Proficiency with electronic health record systems or willingness to learn quickly
