LTC Pharmacist Location: Oklahoma City, OK 73134 Pay: $65+ per hour Schedule: Monday–Friday with rotating shifts Shifts: 8:00 AM–5:00 PM, 8:30 AM–5:30 PM, or 9:00 AM–6:00 PM On-Call: Friday 5:30 PM through Monday 8:30 AM, typically once per month Travel: 0%

Position Overview We are seeking an experienced LTC Pharmacist to support medication management and pharmacy operations for home care patients. This role works closely with physicians, nurses, patients, families, and other healthcare professionals to ensure safe and effective medication therapy.

Key Responsibilities Evaluate and monitor drug therapy based on each patient's overall clinical needs.

Review, receive, and verify new patient medication orders and referrals.

Collaborate with physicians, nurses, customer service, and distribution teams to coordinate new admissions.

Assist healthcare professionals with developing patient care plans and therapeutic objectives.

Establish appropriate parameters for monitoring medication safety and effectiveness.

Conduct initial and ongoing assessments to identify actual or potential drug-related problems.

Monitor patient outcomes using laboratory data, clinical information, and other relevant records.

Provide medication-related consultation to patients, families, physicians, nurses, and other healthcare professionals.

Maintain accurate documentation of clinical activities and patient communications.

Process and verify medications, supplies, and equipment prior to dispensing.

Compound sterile and non-sterile medications, including parenteral nutrition, antibiotics, chemotherapy, hydration, and narcotic analgesics, as permitted by state law.

Follow appropriate aseptic techniques and maintain compliance with USP and USP standards.

Ensure clean room areas and related equipment are properly cleaned, sanitized, and maintained.

Complete required pharmacy logs and documentation.

Oversee pharmacy technician functions as required by applicable Pharmacy Board regulations.

Maintain current clinical knowledge through continuing education, professional literature, seminars, and training.

Follow all applicable company policies, state and federal regulations, and pharmacy practice standards.

Participate in quality assurance and accreditation activities.

Participate in scheduled on-call coverage, including evenings and weekends.

Qualifications Bachelor of Science in Pharmacy required; Doctor of Pharmacy (PharmD) preferred.

Active Pharmacist license in the applicable state(s).

At least 2 years of recent acute-care pharmacy experience OR 1 year of recent home infusion therapy experience.

Experience compounding infused medications.

Experience evaluating and monitoring medication therapy for home care patients.

Strong organizational and time-management skills.

Excellent verbal and written communication skills.

Ability to work independently and collaboratively with healthcare professionals.

Ability to work scheduled on-call evenings and weekends.

Professional society membership and participation is preferred.

Physical Requirements This position requires extended periods of sitting and keyboard use, with occasional standing and walking. The ability to push/pull and lift or carry approximately 10–30 lbs. is required.
