Deliver compassionate, patient-centered care in a hospice setting, collaborating with an interdisciplinary team dedicated to comfort, dignity, and quality of life
Educate and guide patients and caregivers on end-of-life care, symptom relief strategies, and what to expect throughout the hospice journey
Work independently, but never alone
Travel to patient homes within assigned territory

LPN Qualifications

Active Licensed Practical Nurse (LPN) license in the state you work
Two years of LPN experience and the clinical expertise to deliver quality patient care, Hospice experience preferred
Due to the field-based nature of this home health position, a valid driver's license and the ability to spend approximately 20% of your workday driving to and from patient locations are required
