We encourage a culture of teamwork, where physical therapists work alongside physicians, occupational therapists, and other healthcare professionals to provide comprehensive care tailored to each individual.Key ResponsibilitiesConduct comprehensive evaluations of patients, including reviewing medical histories, performing specific assessments, and developing treatment recommendations based on clinical findings.Maintain accurate and detailed documentation of patient care activities, including initial evaluations, ongoing treatment notes, progress summaries, and discharge plans to facilitate clear communication across the care continuum.Work collaboratively with other healthcare professionals to discuss patient progress and modify treatment plans as indicated, ensuring a holistic approach to care.Facilitate effective discharge planning and provide follow-up support to maximize patient transitions from rehabilitation to everyday activities.Engage in ongoing professional development through participation in continuing education opportunities, and serve as a mentoring resource for new or less experienced staff.Participate in local outreach and marketing initiatives to promote our services and the importance of rehabilitation for orthopedic conditions in the community.Patient Population / Client FocusThe ideal candidate will have experience working with a diverse patient population, particularly those recovering from orthopedic injuries, surgeries, or chronic conditions impacting mobility and quality of life.
