This role involves administering medications, monitoring patient conditions, and ensuring high-quality patient care. The LPN will work collaboratively with the healthcare team to support patient recovery and maintain a safe and effective care environment.Key ResponsibilitiesPatient Care & AssessmentAssist in assessing patient physical, psychosocial, and educational needsSupport development and implementation of patient care plansProvide direct patient care under supervision of RNClinical & Medication SupportAdminister medications as prescribedAssist with treatments and patient care proceduresSupport telemetry monitoring and patient observationMonitoring & DocumentationMonitor and record patient conditionsDocument patient information accurately in EMR (Epic preferred)Communicate patient status changes to RN and care teamCare Coordination & EducationAssist in discharge planning and patient educationSupport patient/family learning needsCollaborate with multidisciplinary teamsCompliance & Professional StandardsFollow HIPAA and hospital policiesParticipate in training, meetings, and process improvement initiativesMaintain professional growth and competencyMust-Have QualificationsActive LPN License (State of Iowa or Compact)BLS Certification (Required)Minimum 1+ year of LPN experienceExperience with medication administration and patient careBasic knowledge of telemetry monitoringAbility to work night shiftsPreferred QualificationsExperience with Epic EMR systemPrevious travel LPN experienceExperience in acute care or hospital settingsCore CompetenciesPatient-centered careStrong communication skillsAttention to detailTeam collaborationAbility to work in fast-paced environmentsWork EnvironmentClinical / Hospital SettingNight shift scheduleMay require flexibility across unitsAdditional RequirementsMust be open to contract/travel assignmentBLS + Fingerprinting requiredMust be available for night shiftsMust be open to onsite work in Clinton, IA
